Someone in this context is actually a name. So his mother has 4 sons. North, South, West. The 4th son’s name is Someone. If you still don’t get it, try changing “Someone” to “My” so it becomes My mother has 4 sons.

*The first word that pop into my mind was East* Lol... Answer is Someone
