Hellow po mga mommy ask ko.lng po what if po kung nd ako nakakuha ng MDR mggmit ko pa dn ba philhealth ko nd na po kc ako nakakuha dahil sa ECQ lapit na 37 weeks na dn po tummy ko at extended parin ang ECQ. ano kaya pd gawin.
Nag email rin ako sakanila para sana huminge ng MDR pero ito reply nila, Greetings from PhilHealth! Please be advised that the Member Data Record (MDR) is no longer required in the availment of PhilHealth benefits. This is based on PhilHealth Circular No. 2020-0007 or the Guidelines on the Provisions of Special Privileges to those Affected by a Fortuitous Event(Revision J). Pero try mo po mag email sa costumer service nila. Kasi tru email nakakuha ako ng copy ng MDR 🤗 3days process bago nila ako binigyan ng copy ..

open naman po ang branches ng Philhealth, si hubby po ang pakuhain basta po may dala sya authorization letter , ID and yung form na kailangan fill up-an para makakuha ng MDR, meron po sa online yung form ipprint mo nalang then dalin sa mismong Branch ng Philhealth.

Mamsh na try mo na ba mag register online sa website ng Philhealth? Meron option dun na mag download ng MDR. Ako dun na lang ako kumukuha ng copy eh.. hassle ppunta pa sa Philhealth ofis.
