just call e contact centre. Share wif dem dat u forgotten to ask/get MC. Dey wil ask u if u wanna cum down 2 collect it or mail to your mailing address. Cheers

If past the day already then maybe cannot. Normally you must request on the day itself

Can call the doctor/clinic up, see if you can pick it up cause they have your record.
