Actual age is the time counted from when they were born, but adjusted age is the age they are "supposed" to be if they were born full term. For example, if a baby was born 2 months premature and he is 9 months old, his actual age would be 9 months and his adjusted age would be 7 months. Therefore, he would need to be able to meet the developmental milestones for 7 month olds when he is 9 months of age. Most premature babies catch up to their peers by about age 2 or 3 and look no different. This is a little boy, a triplet who was born at 1.8kg.
