hi, sa mga employed ano po req. ng MAT 2 and sa employer pa din ba magpapasa o ako na ang magpapasa? ung HR kase namen paramg ghost na naman, no reply sa email e. Also, may nkkha bang pera sa mat 2? salamat po.

Refrain from sending documents with unnecessary backgrounds (eg. fingers, hands etc.) PSA copy of your child's Birth Certificate Updated SSS Contribution print out Applicable ONLY to Hires prior June 3, 2019: Complete OB History (accomplished by OB-Gyne, and states the number of pregnancies w/ exact date of delivery/miscarriage) Certified True Copy of Death Certificate or fetal death certificate **for fetal death or is stillborn** Then, if full mo nakuha yung benefit nung nag file ka ng MAT 1. Wala na po makukuha pag na accomplish ang MAT 2.
