my baby 3 months plus. tummy time his head doesn't lift up. is it normal?

Hi mummies. My baby is 3 months plus. I tried to let him do tummy time on bed. He doesn't lift his head up. Is it normal? Last sat when injection . Doctor try to pull his both hands up and his head not able to lift up. Is it normal? I'm worry ?
