Hospital Documents

Hi Mommies! I’m currently 36w5d and preparing na ako ng hospital stuff just in case. Sa documents ito ang nasa checklist ko: Philhealth MDR, Philhealth CF1, Philhealth CF2, Certificate of Payment, Marriage Certificate, Admitting Orders, UTZ results, Lab Results, Mommy book… may namiss pa ba ako ako? Sa SSS ba may need na pa na form? Thanks thanks!Ayaw ko na kasi sana umalis sa hospital si hubby pag nanganak ako para mas madali for everyone.

Mama, youre so organized na! One thing Id add is your ID (any valid ID talaga) kasi kelangan nila for admission, plus a copy of your OBs prescription or referral letter kung may HMO kayo or kung pwedeng i-waive ang deposits, a few of us had that kasi nag-request kami in advance. For SSS, I think okay lang since Philhealth ang primary, pero check din with your OBs hospital kung may specific SSS forms sila gusto, basta better safe than sorry talaga! Also bring extra copies ng lahat because hospitals always need duplicates, haha, and oo na sa wish ni hubby na manatili, most hospitals allow partner to stay or at least stay longer during labor, so ask your hospital about their rooming-in policy para alam nyo na!
