Honestly, the best way to know exactly what your hospital needs is to call them directly or visit their admissions office kasi bawat hospital may slightly different requirements pa, pero usually kailangan mo ng valid ID, your SSS number, at PhilHealth number mismo lang. For the MDR, CSF, at CF-1 forms, tanungin mo na rin directly sa hospital's billing or PhilHealth liaison office nila kasi sila ang makakatulong kung saan makukuha or if they'll prepare it for you na; a few of my mum friends just went through the process and sinabi nila straightforward lang kung direct mo tanungin ang hospital staff. Mas safe talaga to confirm directly with them rather than mag-rely sa hearsay, lalo na ngayon na updated na ang requirements. Good luck, mama!
