My x husband throw all my children health booklet..how do i get a replacement?

Can check with the hospital your child is born, because I noticed babies' health booklet is issued by the hospital and (it bears the name of the hospital) they have the babies info during birth and etc. Can also check with polyclinic if not, you can call MOH hotline and ask.
