For babies below 6 months who are exclusively breastfed, I've read in Breastfeeding Pinays that there is no need to clean a baby's tongue because breastmilk has the capacity to maintain good flora within the baby's mouth, thanks to its antibacterial properties. If a foreign object is introduced into the baby's mouth, he/she is more vulnerable to bacterial growth because we can never know how clean the foreign object is.
