- Admission Letter from your doctor
- Identity Card (both husband and wife)
- Passport (if you are a foreigner)
- Outpatient appointment booklet (blood tests and scan results)
- Original Marriage Certificate (if you are registering the birth of your baby at the hospital)
- List of things to do and important emergency contact list
