mucus issue

Please my baby of 28 days always choke on her saliva and mucus comes forcefully from her mouth and sometimes food from her nostrils. She is on breastmilk and formula. She is losing weight now. I have her taken to the hospital to extract the mucus but same still happens. Please i am really sad. Help pls.?
