From what I understand, kapag sinabi "1 hour before breakfast," usually means you test bago ka kumain ng breakfast, so yes, on an empty stomach pa. But honestly, ang bawat doctor ay may slightly different instructions depending sa situation mo (pregnancy, diabetes management, etc.), kaya better to ask them directly or call their clinic para ma-guide ka properly. One of my mum friends got confused din dati, and when she asked her OB directly, everything made sense na! Don't worry, it's a simple question, and walang maling tanong when it comes to health.
