I don't have direct experience with Tondo Hospital's checkup system myself, pero I'd suggest calling them directly, they can tell you exactly what documents you need to bring, kung kailangan ng referral, at ano ang process nila for scheduling checkups, honestly mas mabilis yan kaysa mag-antay ng sagot online.
