Hello mga mommies!! Ask ko lang if ano pa ba mga requirements sa MAT2 ipapasa ko na kasi sa HR namin. (Medical Records, Birth Certificate ni baby, Confirmation text ng SSS and SSS Reimbursement Form) Ayan na ba yon lahat? #pleasehelp

yes looks like you covered the needed requirement. not sure if thet still require ob history form, if you have it include that too.
