Week 6 postpartum check up

I’ve received 1 of this document frm the nurse upon discharge frm kkh, they told me that i need to bring this document on that day itself. Inside it they only stated the date but didnt indicate the timing & which place to go. Will they send me a text message soon on the reporting details or i can just walk in on that date itself? Do we need to bring anything during our week 6 checkup? Anybody can help on this? Im a first time mum here.
