Subsidised referral

Hi. May I ask , once we confirmed out pregnancy with polyclinic, they will make a referral to our hospital of choice right? Do I need to follow up and make a call to hospital or wait for their call to secure an appointment? Doc didn't mention earlier if I should make an appointment
