OB history

Help po. Late na binigay ng company ko yung OB history form for Maternity reimbursement ng SSS at hindi ko po alam na required iyon so wala po ako OB history form. I gave birth 3 months ago. Pwede pa kaya ako kumuha ng OB history kahit ganto na katagal? Charity ward po ako nanganak kaya wala akong fixed OB doctor.#advicepls
