Transferring from SGH to KKH

Has anybody tried transferring across hospitals before? Any advice on the process? Do I have to ask for referral letter from SGH and go to KKH to make appointment?

both are under singhealth. now u can book appt using health hub. since they are under singhealth no need transfer letter. unless NUH to KKh, if kkh need any documents, they will request from another hosp.
