If employed, Mat1 at Mat2 ay sa employer ifafile. Pag hindi kay employed, sa app ng SSS. Kailangan magdala ng copy ng ultrasound at Notif form (for Mat1). After manganak yung Mat2 kasama ang certified true copy ng birth certificate ni baby, ultrasound (last trimester) at form ng Mat2. For SSS, kailangan one year before yung due date mo ay may hulog at least 3 months. For Philhealth kailangan updated ang hulog tapos Philhealth number mo lang ang kailangan (ospital, not sure sa lying in). For Philhealth, kailangan 6 months ata bago ka manganak.
