Maternity notification

first step po ang maternity notification or MAT1. requirement po ay form ng MAT1, 2 valid id, at ultrasound. second step, after mo manganak yun namang maternity application or MAT2. requirement naman po is yung form ng MAT1 na may stamp ng sss, valid id, birth certificate, if cs: medical abstract. SSS maternity banefits po tawag sa project na yun ng sss.

nkpagpasa na po ako mam ng maternity notification c employer po nag process mam. accepted na din po ang status ko mam.nasend ko na din po sa employer ko ang mga reqts. nkita ko na din po ang amount of benefit. need p po ba another reqts para sa maternity benefit or yun n po yun.
