Punta ka sa hr mo :) bibigyan ka nila ng form fill outan mo lang dala ka din ng records mo or proof na buntis ka na may edd . Ipapasa mo yun sa knila together with the form then sila na mag proprocess nun for you . 1 month before ng due date mo marereleas na yung check .

Mam si HR po yan. Kindly attach the ultrasound or report sa maternity notif. Once stamped po, ask for a scanned copy para may copy ka rin mam.. :)
