Go to sss.gov.ph. 2. Click member and submit your user name and password. 3. Click inquiry, then sickness / maternity. Click again the sickness and maternity claim info. 4. Click maternity notification and answer necessary details needed. NOTE: - EDD lang yung need nila. - Then IPADALA (bawal kang pumunta sa office nila) sa pinakamalapit na SSS office yung form ng MAT1 and photo copy ng first ultrasound. - Ilalagay sa brown envelop indicated sa likod all capitalized Name and SSS Number and also MAT1 or MATERNITY BENEFIT (Depende to sa guard o kung sino man mag aassist sa inyo) - Then iiwan sa drop box for maternity benefit. Wait for the text ni SSS kung kailan ka papapuntahin for any clarification or ok na yung MAT1 nyo.
