I haven't been under Dr Shilla myself, but my friend was with her at TMC for her second pregnancy and found her quite thorough during consultations, though she said the clinic can get a bit packed during peak hours. On the HL question, I think it really depends on your individual circumstances and how your pregnancy is progressing, so it's worth asking her directly at your next visit what she typically does, since every doctor has their own approach to when they're comfortable signing you off.
