Hi mama! Sa maternity benefit, kailangan mo ng original birth certificate lang, hindi necessarily may picture ng actual birth, ang CTC Live Birth Certificate from PSA yan ang kailangan mo, front and back, para complete ang documents mo. A few of us moms din nag-submit lang ng birth cert itself, walang ibang photo requirements, kaya hindi mo kailangan mag-alala doon! Just make sure complete ang lahat ng details sa cert and submit it with your other maternity benefit requirements sa Social Security System or wherever you're applying. Good luck sa claims mo!
