Momsh based sa website ng SSS dapat within 60days after conception makapagsubmit ka ng Maternity Notification Form & Proof of Pregnancy (Ultrasound Report) sa employer mu or if voluntary member sa SSS branch mismo. Please note na dapat meron kang at least 3 months contribution within 12 months prior to delivery to qualify. After ng delivery  magsubmit ka naman ng 1. Maternity Reimbursement Form  2. Duly stamped na Maternity Notification Form rcvd by SSS 3. Certified true or authenticated copy of duly registered birth certificate. 4. Certified true copy of operating room record/surgical memorandum
