Problem in SSS maternity benefit

Hello po! I'm currently employed sa isang BPO, 19 weeks pregnant. Hindi ako makapagsubmit ng mat1 sa company kasi kulang ako ng isang req yung obstetrical history. 2nd baby ko na ito and consulting same OB. Problem is Ob doesn't want to fill up the Obstetrical history ko kasi naclaim ko nmn n daw yung benefit ko nung una (g1) and for g2 e hindi pa naman daw ako nanganganak. My company requires obstetrical history. Twice ko na kinulit si OB na kahit yung g1 lang e sagutan niya pero she is adamant. 🥺😢
