Hey! So from what I understand, ang LCR part (Local Civil Registry) ay dapat ma-fill up ng municipal office mismo pag nag-register ng birth certificate doon, hindi na yan babaguhin once na dumaan sa PSA. A few of us moms went through this recently, and what we found is that kung blank pa rin after registration, you'll need to follow up directly sa municipal health office or civil registry kasi dapat complete na lahat bago mag-PSA. Better to clarify with them now than hassle-ing later pag kailangan mo ng documents for school or CTC, dahil ang PSA copy ay reflection lang ng original na filed sa municipality. Good call checking!
