Is it necessary to apply nappy rash cream on every diaper change? How often should I apply?

Hi there :) My husband and I (1) use a wet wipe and water from a small spray bottle (2) dab the skin with a soft cloth and wipe it as dry as possible (3) apply Desitin cream on the necessary parts For our baby,we saw a little redness and a little tear on the uppermost skin. Since then, we have been applying rash cream after every diaper change. My hubby and I imagine if we are the ones who are experiencing it, it will be painful :( Hence, rash cream after every diaper change will hopefully help a lot :) Hope this helps :)
