HL from SGH experience

From my experience, most doctors will give you HL if you ask for it, especially in the last few weeks, so don't be shy to just bring it up at your next appointment. My gynae didn't volunteer it automatically but once I mentioned I was struggling to commute and work, she was very happy to issue it straightaway. Just let your doctor know how you are feeling physically and they will usually be quite understanding about it.
