From what I've heard, having hypertension in pregnancy doesn't automatically mean doctor will force BTL on you; it's still very much your right to request or decline, and many mums in your situation have successfully negotiated this with their O&G specialist beforehand. Someone in my mum group mentioned that for the third C-sect package at private hospitals, BTL is often offered as a bundled option (roughly RM11,000, 12,000 all-in for the stay), but it is truly optional unless your doctor has a specific medical concern about future pregnancies given your blood pressure history. Since hypertension does carry added risks with each subsequent pregnancy, it would be wise to have a heart-to-heart conversation with your specialist early, ask them specifically whether your current BP pattern changes their recommendation, so you can make the most informed decision for yourself and your family.
