Bleeding

I'm 25 weeks pregnant and yesterday i had trouble pooping. After an hour i went to pee and noticed a bright red blood in the toilet. I dont know if its from my vagina or rectum? 3 hours after i showered and noticed little blood in the underwear. Should i be worried?
