if normal delivery for voluntary members: 1. Accomplished MAT2 form 2. PSA birth certificate or certified true copy of Registered birth certificate at City Hall. 3. Original copy of submitted MAT1 or printed confirmation email from SSS 4. Enrolled personal bank account (under your name) at SSS website and copy of deposit slip. 5. two valid IDs and photocopies.
