If employed, notify ka about your pregnancy sa HR. Provide ultrasound report If self employed or voluntary, notify directly to SSS. Fill up mat1, bring ultrasound report and two valid ID. The maternity benefits will depends on how much is your monthly contribution. atleast 3 months or 6 highest contribution within your 12 month period prior to your semester of contingency ang kasama sa computation
