Yung MAT1 maternity notification po kase yun mommy. MAT 1 (Maternity Notification) – You need to submit this atleast 60 days from date of conception. Also an ultrasound as evidence of your pregnancy or Doctor’s certificate indicating the date of your expected delivery. They will receive the form, record the data and return it to you.

Yes kelangan po ng 2ng valid id mommy. Di kase ako sure if pwede may file ng mat2 if di ka nakapagfile ng mat1. Pero kung di po pwede baka pwede po sa sickness benefit ka makapagfile mommy. Atleast may makukuha ka pa din kahit papano. Sayang nga lalo if updated naman ang hulog mo.
