A.    Child - any of the following: Birth Certificate issued by Local Civil Registry or Philippine Statistics Authority School ID signed by the principal/school head Passport B.    Parent/Legal Guardian Two (2) valid government IDs  Adoption documents - applicable if the account will be opened by the Adoptive Parent Guardianship documents - applicable if the account will be opened by the Legal Guardian *Child under 7 years old - Account must be under the name of the parent in trust for the child. Submit the documents of both parent and child. *Child 7-12 years old -  Account may be under the name of the parent or the child. If under the child's name, may submit only the document of the child.
