Hello po mamsh. Based sa SSS site mismo ito po ang mga kailangan: Mat-2 Requirements (Normal Delivery) Self-Employed, Voluntary and OFW members 1. Mat-2 Form 2. Birth Certificate (Certified True Copy from Local Civil Registrar) 3. Copy of Bank Account (with printed name and account number) 4. 2 valid IDs (preferably government issued) 5. Print-out of Maternity Notification (Mat-1) confirmation submitted ONLINE, BEFORE DELIVERY *Please remember that your bank account number MUST be ENCODED ONLINE before filing. *Fill-up forms properly to AVOID DELAY. *DO NOT FORGET to indicate you CONTACT NUMBER.
