For SSS maternity benefits po, kuha kayo sa SSS branch or download ng MAT-1 form sa website nila. Ipapasa nyo po yung form with your latest ultrasound sa nearest SSS branch. Since preggy po kayo, priority po kayo sa pila. Then ask them po about your contributions, estimated maternity benefits and other requirements needed for claiming po once nanganak na kayo.
