ask ko lng po.. pag gaamitin ang philhealth dpat po complete requirements na bago pumunta ng ospital? currently employed po ako..ano2 po mga requirements? thanks po sa sasagot.

If currently employed, hingi ka sa employer mo ng CSF at CF1 form na may signature nila, hingi ka na din ng certificate of contribution. Ikaw naman, prepare mo yung MDR mo.
