SSS concern

Yes momsh, fill out ka ng Mat1 form & attach mu yung ultrasound report. Tapos ipapa-recieve yun ng HR staff nyo sa SSS, then ibabalik nila sayo yung may SSS acknowledgement stamp or ilalagay sa file mu. Usually aabonohan nila yun bago ka manganak/magleave. Pagbalik mu sa work, mag fill out ka naman ng Mat2, attach ka ng certified true copy ng birth certificate ni baby & operating room record, para maka reimburse naman ang company nyo sa SSS😉
