Honestly, I'd suggest you go directly to the SSS office para sure with the process kasi babago-bago yung requirements, or check their website/hotline muna para hindi ka hassled na mag-uwi ng kulang na documents. One of my mum friends did her contribution update last year and sabi niya straightforward lang naman, kailangan lang ng ID at proof of income, pero better na tawagan mo sila or mag-visit ka ng branch nearest sa inyo para accurate info.
