Oh dear, nipple thrush memang sangat tak tahan, you have my full sympathy! From what I've heard, the usual approach is to treat both mum and baby at the same time, because if only one is treated, you'll just keep passing it back and forth; for mum, miconazole cream applied sparingly after each feed is commonly used, and for baby, a miconazole oral gel in the mouth a few times a day. My neighbour went through this and her doctor prescribed an oral fluconazole tablet as well when the cream alone wasn't enough, and she said relief started to come within a week or so, though full healing took closer to two weeks. Do make sure latch and attachment are also checked, because poor latch can cause similar pain and sometimes thrush is over-diagnosed, so it's really worth seeing a doctor or lactation consultant to confirm before starting treatment.
