Para sa CTC (Certificate of Recognizing the Child), you'll need the baby's birth certificate, your valid ID, and the father's valid ID din if he's listed. Best approach is mag-go ka personally sa Civil Registry office where your LO was born para malaman mo exactly anong dokumento ang kailangan nila, kasi minsan may additional requirements depending sa municipality, a few of us moms had different experiences sa different areas kaya better safe na mag-verify ka directly.
