Ate, I feel you, this can be confusing talaga, especially pag may international setup involved! So when nag-deliver kami sa ospital, hiniling nila ang valid ID ng mama at puwede ring i-present ang father's info or ID if siya available, pero honestly ang process ng COLB sa ospital mismo ay basic lang, ibibigay ka ng Certificate of Live Birth doon na pwede mong gamitin for PSA registration.
Now pag gustong i-register ang father's last name sa PSA kahit hindi pa kasal, kailangan ng affidavit of acknowledgment from the father (pwedeng i-notarize), at kung nasa abroad siya, pwedeng gawin niya yan through the embassy or consulate dito sa Pilipinas, maraming kaibigan ko na ginawa nito na ganito ang setup. Best talaga na ma-consult mo directly ang PSA or ang vital records office ng hospital kung saan ka magde-deliver para alam mo exactly kung ano ang kailangan based sa situation ninyo, kasi may slight variations depende sa location at circumstances.
