Hindi ko pa personally na-file ang MAT 2 case sa SSS, but a few of us mums have been through the maternity benefit process and it can be a bit confusing talaga! From what I've gathered from other parents, you'll need to go to your nearest SSS branch with your required documents (ID, SSS number, medical records from your OB) and ask specifically for the MAT 2 form, they'll guide you through the requirements once you're there. Honestly, the SSS hotline (1547) is super helpful din, and they can walk you through it step by step, kasi bawat case minsan may slight differences depende sa situation mo.
