Can my LO still get to go for his immunization if forget to bring health booklet?

I would think so as the booklet mainly is just checklist for us to tick ourselves. At hosp/clinic the nurses also have the checklist on their computers n will just ask verbally (at least that’s what happened for me) can bb do this n that?
