Ano ang mga requirements para mag apply ng philhealth indigent?

Honestly di ko na masyadong updated sa exact requirements ngayon kasi nag-change na rin ng procedures, pero alam ko need mo ng birth certificate, proof ng residence, at ID ng parents, best move is pumunta directly sa nearest PhilHealth office or municipal health center kasi sila ang sure na guide mo step-by-step, especially since first time mom ka pa! One of my mum friends, she just brought documents na kaya niya and asked the staff what else need pa, way easier than mag-guess online.
