Go to https://vaxcert.doh.gov.ph/ Make sure you have your Vaccination Card and any Government-issued ID before you continue. Review the data privacy statement. Check the box with I have read and agreed to the Privacy Statement Enter the Details needed. Answer all the questions being asked. After answering the questions, click the verify vaccination information. Check the details that are being put. Then press continue. Enter the 7 characters shown below. Note: Copy As Is Shown. Then if Record Found Congrats! If you will use it for travel click the box and enter your passport number. If not for travel, you can Generate a Vaccine Certificate by clicking the GENERATE VACCINATION CERTIFICATE.
