Momsh, I haven't had a 4th CS myself pero I have a few mum friends who've gone through multiple cesareans and honestly, mas mahirap talaga ang recovery each time kasi ang body mo ay nag-scar na from previous surgeries. One of my friends said ang third and fourth CS niya ay medyo mas matagal ang healing compared sa first two, at mas sensitive ang incision area. Definitely talk to your OB about this kasi they can assess your scar tissue and give you the best plan para sa safe delivery, it's really important na ma-monitor mo closely especially with multiple CS.
